FR Corrections Ideas/Suggestions
Review the letter that you receive in titled - "Necessary Action Requested". On this letter the SDRC's HCBS Team will have identified Federal Regulations (FR) that we need additional information regarding or changes that need to be made to your agency to meet HCBS Compliance. Review the FR on the letter to which number needs additional support and review the suggested items to show compliance.
Once you have implemented the suggestion to your program, please send in proof to hcbs.frule@sdrc.org for review by the SDRC HCBS Team.
FR1: Access to Community
Calendar of events that the individual served can choose to attend.
Individual can access their community with natural or paid supports.
FR2: Choice of Setting
All individuals served as a current Person-Centered IPP and (if applicable) Addendum on file.
FR3: Rights & Responsibilities
Policy & Procedures of agency that shares / educates the individual served their client rights.
Photo of Client rights that are posted in common areas.
FR4: Independence
FR5: Choice of Services & Support
Policy & Procedure showing how an individual served can pick who supports them during their time at that agency.
Client notes of offering all their options of service & support via the ISP or the IPP/Addendum showing options (including non-disabled options).
FR6: Admission Agreement
Each Resident has a Admission Agreement that is signed by legal representative
FR7: Privacy
Policy & Procedure on how shared rooms are determined.
Roommates are agreed upon and how conflict is resolved.
Photos of each bedroom door with keyed entry & bathroom with twist knob.
FR8: Access to Food & Choice of Schedule
Policy & Procedures of how menus/food decisions are created
Removal of any Rules regarding where the Resident can eat food.
Policy & Procedures on how to support Resident on their daily activities.
FR9: Right to Visitors
Copy of updated House Rules removing visitor policies.
Implimentation of how Visitor Policy is agreed upon by the Residents.
FR10: Accessibility
Photos of accommodations developed specific for residents.
